Privacy Policy

This policy applies to www.balancedhealthlb.com and to the forms, booking links, and analytics on it. It does not replace the Notice of Privacy Practices that governs your medical record as a patient of the practice — that notice is provided at your first visit and is available on request.

Our contact and consultation forms ask for your name, email address, phone number, the service you are interested in, and anything you choose to write in the message field.

Please do not send detailed medical information through this website. The forms on this site are for scheduling and general inquiries. Clinical questions are answered by a provider through a secure channel once you are a patient.

Like most websites, ours records technical information when you visit: IP address, browser and device type, referring page, the pages you view, and the approximate region you are browsing from. We use this in aggregate to understand traffic and to keep the site working.

We use Google Tag Manager and Google Analytics to measure how the site is used, and we may use advertising cookies from Google and Meta to measure the performance of our ads and to reach people who have visited the site. These tools set cookies in your browser.

You can block or delete cookies in your browser settings. You can opt out of Google Analytics with the Google Analytics opt-out add-on, and manage ad personalization at Google My Ad Center.

This site is protected by Google reCAPTCHA so we can tell real visitors from automated abuse. reCAPTCHA collects hardware and software information and behavioral signals and sends them to Google for analysis. Its use is subject to the Google Privacy Policy and Google Terms of Service.

To respond to your inquiry, schedule and confirm appointments, answer questions about services and pricing, send you information you asked for, improve the website, and meet our legal and record-keeping obligations.

We do not sell your personal information.

When you give us your phone number we may call or text you about your inquiry, appointment, or treatment. Message and data rates may apply. You can stop texts at any time by replying STOP, and you can leave our marketing email list using the unsubscribe link in any marketing message or by contacting us directly. Appointment and care-related messages are not marketing and may continue while you are a patient.

We share information with the service providers who make the business run — our online booking and practice software, website hosting, email delivery, and analytics providers — and only so they can perform that work for us. We may also disclose information where the law requires it, or to protect our rights, our staff, or our patients.

Booking is completed on our scheduling provider’s own platform. Once you click through to book, what you enter there is handled under that provider’s terms as well as this policy.

Website inquiries are kept for as long as we need them to answer you and to keep a reasonable business record. Medical records are retained for the period required by New York State law and are governed by the practice’s Notice of Privacy Practices, not by this page.

You can ask what information we hold about you, ask us to correct it, or ask us to delete information we are not required to keep. Write to info@balancedhealthlb.com and we will respond within a reasonable time. Requests about your medical record follow the process set out in the Notice of Privacy Practices.

This site is served over HTTPS and form submissions are transmitted encrypted. No website can promise perfect security, so please use good judgment about what you send through a web form.

Our services are intended for adults. This website is not directed to children under 13 and we do not knowingly collect their information. If you believe a child has sent us information, contact us and we will remove it.

This site links to third-party sites, including our booking platform and our social media pages. We are not responsible for their content or their privacy practices.

We may update this policy from time to time. The date at the top of the page shows when it last changed, and the current version always lives at this address.
